How are the EOL tools and practices alive in your life and leadership right now? Describe any practices and core routines you draw from on a daily or weekly basis that are most important to you.

Are there any conditions or personal needs that you feel are important for the facilitators to know about, or current life circumstances that might prevent you from fully participating in the program? Please include any psychiatric conditions, medical conditions, or emotional or chemical imbalances for which you have been diagnosed, and if so how you are being supported in working with this (e.g. therapy, medication, exercise, etc.). We will hold this confidential.

As a member of the Facilitation Team, you are committing to participate in all sessions of the program for which you are applying. You are responsible for reviewing in detail the dates and times of sessions before submitting your application. By submitting your application, you are committing to being present for all sessions. In addition, please consider your availability to engage with buddies, POD, and assignments for roughly 6-10 hours of time each month. Can you foresee any challenges meeting these requirements? a. All program weekends b. A full day pre-launch retreat 9 am – 9 pm in Bolinas the day before each program starts. c. Pre-weekend facilitation team meetings the evening before each weekend starts, from 6 – 9 pm. d. Post-weekend debrief meetings every Sunday evening from 6-7:30 pm e. One facilitation team conference call between each EOL weekend f. Support of a group of 5-6 EOL participants between the weekends (format TBD, but may include making yourself available for conference calls, and email / phone correspondence)
